Once a record is validated, the Name of the Deceased field and the Date of Death field are locked.

In order to amend either field please follow the steps below: 

1. From within Tab A, click on the red circle icon beside the validate box
2. Enter the revised data
   
3. Click OK and select Yes
4. Click on the Send Revalidation request by email button.
   
   
5. The email will populate with all necessary information - please do not remove any information from what populates or the revalidation cannot be completed.
6. Once the information is received, you will receive the activation code in response to your email.  To apply the activation code, simply follow steps 1 through 3 and enter the activation code into the appropriate box.
